"The Plication Suture only technique" in treatment of Penile deviation

(AN INNOVATION TO THE STANDARD PROCEDURES)
Shahryar Cohanzad,MD (Sex-Urologist)

"Penile deviation" has become increasingly important in recent years;‎‎‎‎‎‎ this is not because the disease occurs more frequently. In fact the elimination of "Taboos" on this subject in the population and mass media has made it possible for patients to talk more easily to physicians about

this problem and hence, much more diagnoses.
Among all procedures been introduced and evolved, "Nesbit and its modifications" has been the most popular one. One of the major shortcomings attributable to this technique, is the inevitable injury to the very much vulnerable "Tunica-Corporeal" structure;‎‎‎‎‎‎ which indeed might have a detrimental effect upon the intricated integrity of the erectile capacity.

Ob‎jectives:
It really looks rationally ideal to have the deviated penis straitened without imposing the patient any potential impairment to both anatomical and functional integrity of erectile capacity. On the other hand, it is quite evident that even in experienced hands;‎‎‎‎‎‎ the preset straitening procedures are almost always associated with injuries at least and at some extent to the erectile structures. From this particular point of view, introduction of a safe, practical, time sparing and equally effective technique would be of great benefit and paramount importance.

Materials:
From 1997 through 2001, eight men between 19 to 57 yeas of age, fully consent and aware of the potential shortcomings, underwent "the plication suture only technique" for treatment of their deviated penes.
Six men had "Peyronie's D."and in two others, there were "Primary Penile Deviations"

Methods:
For utmost result and satisfaction, the plication sutures must be applied precisely at the "Exact counter-point" of the deviation. This mandates either an artificial or vasoactive induced erections just prior to the application of the aforementioned sutures.
As illustrated below, the plication suture is one of the kinds regarding the direction through which it is guided. This undermines the "nonabsorbable knots" giving an excellent smooth surface to the "Corpus Cavernosum" as the final result.

Results:
The outcomes of the procedure have been enormously satisfactory in all eight patients. None met any complication thus far, nor expected to have one.
It looks rationale to have the patients at least with "mild to moderate" penile deviations undergone this procedure particularly because of the "ease, safety, time sparing and equally effectiveness" of the technique in comparison to the more popular standard ones.
